Special Enrollment Opportunity - May 2024

Currently enrolled families may complete this form to enroll a child or children (13 years old and younger) in the BSK program for summer care or ongoing enrollment. Requests are due by May 27th, provider selection by July 11th, and care starting by July 26th, 2024. This temporary opportunity is open only to currently enrolled families and requires following specific timelines and procedures outlined by the BSK program. Please reach out to a BSK Family Support Navigator for more details.

Stay Updated: Opt-In for BSK Program Texts

Families and providers can complete this form to consent to receiving BSK program-wide text messages, ensuring they stay informed about important updates and announcements. Your contact information remains private, and you can still engage in individualized text communication with the BSK program without needing to sign up separately.

Reporting Late or Missed Payments

For families or child care providers to report BSK subsidy payments which have not yet been received.

Family Request to Change Provider

To be completed by families or families with the assistance of a BSK Family Support Navigator when the family wishes to change child care provider.  This includes attending a different site within the same agency.  A minimum of two weeks notice should be given.

Summer Care – Family Request for Change in Provider and/or Change in Authorized Amount of Care

To be completed by families or families with the assistance of a BSK Family Support Navigator.  We ask all families to complete this form by June 1st.  Adjustments in amount of authorized care are not automatic for the summer.  Families must report these changes in advance.  Changes in provider, even for short periods of time, must be requested.  Only licensed providers are eligible for BSK subsidy.

Summer Care – Provider Report of School-Age Tuition Rates for Summer

To be completed by providers by May 15.  Providers may charge different rates for summer care, as long as these are their published school-age tuition rates for the summer period (June 13 – September 6, 2024).  No adjustments in school-age subsidy payments can be made without notification from the provider of their summer rate structure.

 

Family Request for Change in Amount of Authorized Care

To be completed by families or families with the assistance of a BSK Family Support Navigator.  Families may request to change their amount of approved care within the following categories: part-time (up to 20 hours per week), full-time (21-50 hours per week), and extended care (more than 50 hours per week).  Requests for extended care require supporting documentation.  A minimum of two weeks notice should be given.

 

Family Request to Add an Infant or Other Eligible “New” Child

To be completed by families or families with the assistance of a BSK Family Support Navigator.  This form is for families currently enrolled who wish to add a child.  This option is limited to children less than 12 months of age and on case by case approval for unique circumstances such as, gaining new custody of a child not previously part of the family’s household, or children with medical, behavioral, or developmental needs.  Families must give at least 2 weeks notice of the requested addition.

Payment Statement Request

Families and Providers may request a statement of BSK payments made by BrightSpark.  Families may request a copy of all or a segment of payments made on their child’s behalf.  Providers may request a statement of all, or a segment, of payments made to their child care program.

1099 Form Request (2023)

 

Providers may request a re-send of their 1099 form from BrightSpark Early Learning Services.  This will contain any BSK subsidy payments made by BrightSpark, which were issued in 2023.  Providers seeking a 1099 from Scholar Fund should email bsk@scholarfundwa.org.

 

Family Request for Re-Evaluation of Approved Subsidy Rate (COMING SOON)

To be completed by families or families with the assistance of a BSK Family Support Navigator.  Families experiencing a significant change in their economic well-being may request a reassessment of their financial status, which may result in a change in subsidy award.  Documentation will be required.  Families may also choose to submit this form when there is a change in the number of household members.  If the reassessment determines the family is likely to be eligible for WCCC subsidy, the family will be asked to obtain a WCCC denial letter within 60 days in order to continue their BSK subsidy.
